8/30/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Client upon arrival to the home setting. Upon arrival to the home setting, Client appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She stated to QMHP-C that she was in a good mood. QMHP-C informed Client that this session was going to consist of a family session. Mother reported she continues to struggle with Client being able to follow directives and communicate with her. 2. Behavior: During the second portion of the session, Client appeared to be a little irritated with her mother as well as QMHP-C when discussing ways to control her behavior to prevent her from getting in trouble in the home daily. 1.Intervention: QMHP-C began session by inquiring about Clientâs current thoughts, feelings and emotions. QMHP-C engaged Client and mother in open dialog to allow them to verbalize interactions and behaviors since last session. QMHP-C engaged Client in therapeutic activity on communication skills and being more assertive. QMHP-C role-played the scenarios with Client and her grandmother to practice communication skills and how to display respectful behaviors. QMHP-C processed positive and negative feelings about the exercise. 2.Intervention: QMHP-C discussed following rules and regulations and ask mother about how often she has to talk with Client before she listens. QMHP-C talked with Client about expressing her thoughts and feelings in a more constructive manner through more constructive actions. QMHP-C discussed sibling rivalry and alternative measures to confrontation and being argumentative with each other. QMHP-C discussed family dynamics and assessed environmental stressors. 1.Response: Client and Ms. Hodges appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Through the role-plays Client and grandmother were able to recognize positive and negative responses and reactions toward each other. Client struggles with talking back and not respecting her grandmother's limitations. 2.Response: Ms. Hodges verbalized that she has to constantly talk with Client and redirect her on several occasions daily because she knows that she misses her mother. Ms. Hodges as well as Client reported that when Client gets upset when she do not have her way and sometimes walks away from her. Client admitted that she gets upset immediate after being told "no".
